Student-Athlete Forms
All student-athletes who intend to participate in intercollegiate athletics at Lakeland University must have the following completed prior to participation:  Physical exam for First Year athletes (Freshmen and Transfer student-athletes physicals must be signed by a MD or DO) and Junior Year athletes.  A thorough medical history, and demonstrate proof of insurance that covers athletic injuries. The NCAA now requires insurance coverage of $90,000.  All of this information must be on file within the athletic training department prior to participation.
 
Information must be completed and returned to Lakeland University by August 1, 2018.

Welcome to the 2018-2019 Lakeland University Athletic season.  We once again have SportsWare as our online data entry system.  Below you will find the instructions for access and use of SportsWare.  We have broken the instructions into First Year (Freshmen and Transfers) and Returners.

The following is a quick reference/check list for what paperwork you will need for athletic clearance.
SportsWare information 
  • All tabs completed in “My Info” section
    • (General, Address, Emergency, Insurance, & Medical) & profile pic (“head shot”) uploaded 
  • Picture of front and back of current insurance card uploaded (at bottom of Insurance tab)
  • Completed medical history section
  • Completed and submitted Statement of Insurance and Medical Access form
  • Completed and submitted Consent, Medical Release, and Concussion Information form
  • Pre-participation Physical Exam (PPE) form (freshmen/transfers/new student-athletes & Junior eligibility athletes)
  • ADD/ADHD medication form and physician documentation (for those taking ADD/ADHD meds only)
Concussion and Sickle Cell Education form (Will be completed with submission upon completion of attached PowerPoint)
Note:  The Sports Medicine staff will check the paperwork and mark you cleared once everything needed is completed.
 
Also, you will receive some type of communication from the University/Health Services about Insurance.  Please make sure to complete that information.  The forms for Athletic Insurance and University Insurance are two separate things.  If you do not complete this information you risk being charged for insurance through the University.
